About the Role

VBHC Care Coordinators provide person-centred support that maximizes coordination of care and minimizes duplication of effort. This work will support the persons care coordination plan that is built on their goals and aspirations. The Care Coordinator shall work closely with the key person, the Nurse Unit Manager and the wider multi-disciplinary team to achieve the right service at the right time, by the right people and in the right place to maximise a seamless and safe transition out of acute care and follow their journey with the GP into the community. This shall be in the evolving context of the Values Based Health Care model at Central Gippsland Health.

The VBHC Care Coordinator will work closely with the participant to enable them to have optimal quality of life, improve their health outcomes and remain active in the community. This will be achieved through regular monitoring, close linkages and relationship with the GP and delivering a care plan that focusses on outcomes that matter to the participant.


Undertake comprehensive person-centered assessments that focus holistically on the patient's needs and goals for a safe discharge from the acute setting and follow their journey back into the community.

Work in conjunction with the Nurse Unit Manager, Multi-disciplinary team, and the participant's GP.

Provide advocacy and support the patients/participants

Tailor supports and goals to suit patient's health activation level

Contribute to a collaborative and respectful partnership with the patient, understanding the contribution that the person can make to their own health.

Qualifications, Skills and Experience
